CNA

Job Summary:
Provides care and emotional support to residents under the direction of a licensed nurse, observes and reports on changes in conditions, and assists families and visitors.

Job Duties and Responsibilities:

  • Provides physical care to residents according to established procedures and performance standards, including, but not limited to:  Oral Hygiene, Bathing, Incontinence care, Positioning, Shampooing, hair care, Toileting, Making beds, Obtaining weight, temperature, pulse, Ambulation, Perennial care, Skin care, Dressing, Nail care, Range of motion exercises, Use of adaptive equipment, Feeding assistance, Applying & checking restraints, Transfer.
  • Responds to resident call lights and requests.
  • Maintains safe and sanitary halls, resident rooms, closets, bathrooms, tub/shower, utility rooms, dining rooms and lounge areas consistent with established standards.
  • Accurately completes required documentation.

Education and Training:
Successful completion of Certified Nursing Assistant training, resulting in State certification.  Must be currently on the Minnesota Nursing Assistant Registry.

WARD ASSISTANT

Job Summary:
Provides assistance to residents under the direction of certified nursing assistants and a license nurse.  Promotes appropriate resident behavior, provides emotional support and observes and reports on changes in conditions.  Greets and assists families and visitors.

Job Duties and Responsibilities:

  • Responds to resident call lights and requests and report to a CNA or Team Leader if a resident needs hands on care.
  • Pass ice water (with prior training and knowledge of Thickened Liquid Policy).
  • Make beds.
  • Restocking Linen Carts and Linen Dumps.
  • Assist with transporting residents to activities and dining room.
  • Maintain safe and sanitary halls, resident rooms, closets, bathrooms, tub/shower,

FLOOR NURSE TEAM LEADER - LPN

Job Summary:
Team leader provides nursing care to assigned residents, including medications and treatment administration, documentation and other therapeutic interventions under the direction of an RN. Maintains order during shift, including supervising resident aides, medication technicians and monitoring the appropriate care.

Job Duties and Responsibilities:

  • Knowledgeable of resident conditions at all times.
  • Abuse Prevention:  Responsible for the implementation of Bayshore Abuse Plan including unit staff education and compliance, complete investigation and Vulnerable Adult Reporting.
  • Provides nursing care according to physician’s orders and in compliance with established standards, administrative policies and the resident plan of care.
  • Assesses and reports resident change in condition to the physician, designated representative and to supervisor and initiates necessary follow-up to assure resident welfare.
  • Maintain accurate and complete records of nursing observations and care according to company policy and procedure.
  • Administer meds and treatments, including PRN meds and treatments and re-orders such under the direction of the physician and company policy and procedure.
  • Maintains order, cleanliness of medication room and/or treatment room and re-orders necessary supplies according to policy and procedure.
  • Orders lab work, x-ray and outside supportive services/therapies per physician orders.
  • Assists in the transcription of physician orders per policy and procedure.
  • Assists in pre-checking, double-checking of all routine physician orders and/or monthly cardex orders per policy and procedure.
  • Accurately completes medical records during shift, e.g.:  nurses’ notes, admission, transfer, death, incidents, meds and treatments.
  • Assists in supervising, evaluating, and disciplining Nursing Assistant and/or Medication Technicians.
  • Communicates with Nurse Manager/Supervisor regarding change in resident condition, employee concerns, family concerns & facility structure.
  • Participates in maintaining the safety of the facility, including participation in fire/tornado drills and first aid training per policy and procedure.
  • Participates in in-services/meetings both in-house and outside the facility.
  • Maintain good working relationships with staff, supervisor and other departments.
  • Assures that each resident is treated with dignity and respect and that confidentiality and the components of the Bill of Rights are maintained.

Education & Training:

  • Graduate of a State approved School of Nursing.
  • Currently licensed as an RN/LPN in the State of Minnesota.

Work Experience:
New graduate acceptable.  Previous experience in hospital or long-term nursing care preferred.

NURSE SUPERVISOR

Job Summary:
Under the direction of the Nurse Manager assumes shift responsibility for primary care coordination of resident/family, including but not limited to, compliance, POC, quality of life assurance, interdisciplinary coordination as needed, staff resource and support. In the absence of Nurse Manager, is responsible for overall facility operation for shift including supervision of support staff. Assures interdisciplinary intervention to assist residents in achieving maximum level daily.

  • Communicates with the DON and Adm. any pertinent info. Regarding changes in resident condition, physician calls, employee problems, and problems with physical structure in timely manner.
  • Sees that employment policies and procedures are used effectively in nursing department including initiation and documentation of disciplinary action as needed.
  • Assures that all policies and procedures of BayShore Health Center are being adhered to.
  • Participates in maintaining the safety of the facility, including participation in fire/tornado drills and first aid training per policy and procedure.
  • Participates in inservices/meetings both in-house and outside the facility.
  • Maintains good working relationships with staff, supervisors and other departments.
  • Assures that each resident is treated with dignity and respect and that confidentiality and the components of the Bill of Rights are maintained.
  • Adheres to all infection control policies and procedures. Establishes and participates in activities for assigned residents.
  • Completes self-scheduling with interest of facility needs, as well as own, in mind.
  • Performs other related duties consistent with operational needs as assigned.

Education and Experience:             

  • Graduate of a State approved School of nursing.
  • Currently licensed as an RN in the State of Minnesota.
  • Previous management experience in long-term care nursing preferred.
